Selective Mutism

What It Is

Selective Mutism typically emerges in early childhood and is characterized by consistent failure to speak in specific social situations, like school or public places, while the child speaks normally at home or with close family. It’s not a speech disorder but a psychological condition that requires careful, patient intervention.

Identifying Issues

Children with Selective Mutism may avoid speaking in school, social groups, or unfamiliar environments, often due to extreme anxiety or fear. This can impact academic performance, social relationships, and emotional well-being. Early identification and supportive strategies are critical for success.

How We Can Help

At Bjorem Speech®, we support children with Selective Mutism through the DIR® SM Program, a comprehensive, relationship-based approach that considers the whole child. Selective Mutism is an anxiety based disorder that often includes sensory, motor, and language challenges, making individualized support essential. The DIR® SM model helps identify a child’s stage within the Functional Emotional Developmental Capacities (FEDCs) speaking hierarchy, ranging from nonverbal to partially verbal to fully speaking, and guides them through developmentally appropriate, play based interactions that promote comfort, regulation, and expressive language. Using tools like the Systematic Sound Sequencing Strategy (SSSS), we meet children where they are, supporting communication through body sounds, environmental sounds, and gradual speech production.

Parents play a vital role in the process. With guidance and coaching from a speech-language pathologist, caregivers can use DIR® SM strategies at home to support their child’s progress in everyday environments. By creating safe, low-pressure opportunities for connection and communication, parents can help bridge gains made in therapy to real life. With a strong focus on emotional development, individual differences, and relationship building, we help children progress from silence to confident, meaningful communication in the home, school, and community settings.
